I thrive on feedback.  It's the only way I know the stories I work so hard on are being read. Still I know many people either don't have the time to write an email or prefer to remain anonymous.  To help in situations like this I've created this Rapid Feedback survey form.

It's designed to be FAST and EASY.  All you need to do is 'Point and Click', key a comment or two, and an email is automatically generated directly to me.  It can even be anonymous.
